The Hal Saflieni Hypogeum in Paola, on the main island of Malta, is a Neolithic underground burial and ritual complex cut entirely from soft globigerina limestone between roughly 3300 and 3000 BCE, during Malta's Temple Period. It is the only known prehistoric underground temple in the world, and it was inscribed on the UNESCO World Heritage List in 1980.

Underground Neolithic burial complex of chambers on three levels, cut from the limestone about 5,000 years ago.

The complex descends across three levels, with chambers, passages and niches carved with stone tools, and it is believed to have begun as a natural cave system that later generations enlarged over many centuries. Archaeologists estimate that the remains of several thousand people were deposited here, which indicates long use as a communal necropolis. The site was discovered in 1902 during building work on the surface, and early excavations were carried out in the first decades of the twentieth century. Among the best-known spaces are the Oracle Room, where a carved recess is known for the way a low voice resonates through the chambers, and the Main Chamber and Holy of Holies, whose false doorways, trilithon-style openings and corbelled ceilings imitate the above-ground megalithic temples. Traces of red ochre decoration, including spirals and honeycomb patterns, survive on some ceilings. Finds from the hypogeum, including the Sleeping Lady figurine, are held in the National Museum of Archaeology in Valletta rather than on site. The visit is a timed, guided experience in small groups, because carbon dioxide, humidity and temperature changes can damage the rock and its painted surfaces. Heritage Malta manages the site, and a purpose-built visitor centre with environmental controls limits daily numbers and the time spent underground. Conservation work has stabilised the structure, and tickets are released well ahead of the visit, so advance reservation is part of the experience in southeastern Malta.
