The region was inhabited by several indigenous groups including the Chorotegas in the Nicoya Peninsula, the Brunkas (Boruca) in the south, and the Térraba and Brunka peoples in the Osa Peninsula and southern zones. The Nicoya Peninsula was a culturally advanced region with sophisticated ceramics, goldwork, and trade networks extending to Mexico and South America.

Christopher Columbus sailed along Costa Rica's Caribbean coast in 1502, but the Pacific side was explored shortly after. Spanish conquistadors encountered fierce resistance from the indigenous Chorotega people of the Nicoya Peninsula. Unlike much of Latin America, Costa Rica's indigenous population was relatively small and the Spanish found little gold, leading to slower colonisation.

Under Spanish colonial rule, Puntarenas City emerged as the main Pacific port for the region. The port facilitated trade between the Central American interior and Spanish ships. The indigenous population declined dramatically due to disease. The Nicoya Peninsula retained a relatively distinct identity due to its semi-isolation.

Costa Rica declared independence from Spain in 1821 (and from the Central American Federation in 1838). The development of coffee as an export crop in the Central Valley transformed the country's economy, with Puntarenas serving as the primary Pacific export port until the railway to the Caribbean was completed. This era saw the modernisation of Puntarenas City.

The United Fruit Company established banana plantations in southern Costa Rica (Quepos area and Osa Peninsula) in the 1930s–40s, bringing significant economic development and infrastructure — but also labour conflicts and environmental change. Quepos was effectively built by United Fruit.

Costa Rica emerged as a global leader in conservation, establishing its national park system in the 1960s–70s. Cabo Blanco (1963) was Costa Rica's first protected area. Manuel Antonio (1972) and Corcovado (1975) followed, setting the foundation for the eco-tourism industry that now drives Puntarenas's economy.

Puntarenas province has become one of the world's premier eco-tourism destinations. Manuel Antonio receives over 400,000 visitors annually, Monteverde is a global model for community conservation, and the Osa Peninsula is recognized as one of Earth's last pristine tropical wildernesses. Puntarenas faces ongoing challenges balancing conservation with development.
